No referral is need to access Brain Care services with private health insurance. However, you must check with your provider to assess your eligibility. Once you have been approved, you can make an appointment. Please read below for more details.

Step 1 - Confirm Your Eligibility

Once you call your private health insurance provider, and confirm you are covered for our services, you are ready for the next step.

Step 2 - Gather Your Information

You will need to complete a Client Information Form. You may download it, fill it in, and attach it to your booking request now. You can also print it and bring it to your first appointment. Alternatively, you can fill it in during your first visit to the clinic. You can download the form here.
