Bookings - Employee Assisted Program

Brain Care is affiliated with a range of EAP providers to help support the community. If your workplace is associated with one of these providers, you can use this program to receive a referral. It is important to note that all information is confidential and will not be shared with your employer unless you provide written consent.


Step 1 - Identify your workplace EAP provider

You will need to ensure your EAP provider is compatible with Brain Care. We are registered to work with Life Works, Converge and Benestar amongs others. If you don’t see your provider listed, please contact us for further information.

Step 2 - Get a referral

Getting a referral is as easy as calling the EAP provider (not your employer) to assist with finding a clinician. This referral will be submitted to Brain Care and you will be contacted by our team to arrange an appointment shortly after. The amount of sessions provided under your EAP may vary.

Step 3 - Gather your information

You will need to complete a Client Information Form. You may download it, fill it in, and attach it to your booking request now. You can also print it and bring it to your first appointment. Alternatively, you can fill it in during your first visit to the clinic. You can download the form here.
