Bookings - Workcover
Some of the clinicians at Brain Care are registered to provide consultations under WorkCover claims, both in Queensland and New South Wales. Please contact us to check availability and appointment times.
Step 1 - Identify your Workcover Details
You will need two items:
Insurance Claim Number
Case Manager Details
Step 2 - Gather your information
You will need to complete a Client Information Form. You may download it, fill it in, and attach it to your booking request now. You can also print it and bring it to your first appointment. Alternatively, you can fill it in during your first visit to the clinic. You can download the form here.
